June Simmons
June Simmons

As founding President and CEO of Partners in Care Foundation, June Simmons strongly believes that health care and social delivery of services need to change, especially in community and home settings. Under her leadership, Partners In Care Foundation has grown ten-fold since Its establishment in 1998 and has been awarded many major grants to investigate and evaluate new models of health care and social service delivery, including those in managed care environments. Simmons has worked at the national level to develop evidence-based programs to help address costly issues such as the last year of life, falls for the elderly and enhanced management of chronic diseases. Simmons was selected as a delegate for the 2005 White House Conference on Aging. She also serves as a member of the National Advisory Council on Aging. She also serves as senior advisor to the Alliance for Children and Families' National Initiative for Transforming Social Service Geriatric Practice, a four-year Initiative funded by Atlantic Philanthropies. She also serves as an advisor for the Practice Change Fellows Initiative. Simmons was founding chair of the National Chronic Care Consortium and has many years of experience in bringing positive change to health and community service setting.
